Care Coordinator
Location: Shreveport, Louisiana
Description: Shriners Hospitals for Children is currently interviewing Care Coordinator right now, this occupation will be presented in Louisiana. More details about this occupation opportunity please read the description below. Screens and assesses patient/family psychological, family, cultural, financial, and community status. Identifies patients who receive or who may benefit from SHC and community-based se! rvices.
Coordinates timely, appropriate, preadmission and discharge planning.
Coach’s patient/family in knowledge and skills to become self-sufficient in their own case management, health care system access, negotiation, and navigation.
Acts as clinical liaison with insurance companies to obtain pre-authorization for services needed in the community. Provides clinical information to insurance case managers or primary care physicians.
Job Requirements
Bachelor's Degree in Nursing required. Must be certified within 12 months.
Experience in clinical setting / pediatrics preferred.
Louisiana State Registered Nurse Licensure
Experience with general office equipment preferred. Experience with basic medical equipment required.
