About the Position:
The Home Health Aide is responsible for providing various patient related services to meet the personal care needs and to promote comfort and independence to acutely ill and/or chronically ill patients in their homes. This includes recording clinical and progress notes and coordinating other patient care activities to provide the highest quality of care.
Responsibilities Include:
1. Provide and/or assist with basic personal care such as bathing, oral hygiene, toileting, skin care, ambulation, and range of motion.
2. Measure and record temperature, pulse, respiration, and blood pressu! re as directed on the care plan within the limits of the aide.!
3. Assists with household tasks to keep rooms used by the patient neat and clean and perform tasks directly essential to the patient's health including making beds, changing bed linens, laundry, etc., as directed on the care plan.
4. Provide clean and safe environment within the home including safe use of equipment such as foot stools, side rails, oxygen, wheelchairs, walkers, and hoyer lifts.
5. Performs ROM and other simple procedures for therapy as directed by a therapist as an extension of therapy services.
6. Reports observations of the patient's condition to the designated supervisor or on-call nurse.
